Our Story
With over 21 years experience of Physical Therapy serving the greater Elko NV area.
New Patients
- Fill out the “New Patient Form” prior to your first appointment.
- Arrive 10-15 minutes before your scheduled appointment to fill out paperwork and/or check in.
- Wear or bring comfortable, loose-fitting clothing to make it easer to get to the area being treated.
- Bring your referral for physical therapy from your physician.
- Bring your insurance card and a form of ID.
- Each treatment will be around 45-60 minutes.
Billing Information
- Fallon Physical Therapy will bill your insurance carrier as a courtesy for you; however, insurance co-payments are to be paid at the time of services rendered and it is your responsibility to be sure you are covered for PT visits.
- We participate with most insurance plans and will assist in verifying benefits prior to treatment.
- Payment arrangements can be made with the billing department.
